To that end, here are some frequently asked questions regarding visits to our offices.
How do I book an appointment?
We offer same-day, early morning, late evening, and Saturday appointments.
How do I confirm an appointment?
Vita Fitness & Physical Therapy uses an automated confirmation system to confirm appointments with patients. You will receive an email and/or text message asking you to confirm your appointment.
Please notify us of appointment changes 24 hours before the appointment at the very latest. Just call our office 414-272-8482 (Third Ward) or 414-563-1230 (Wauwatosa).
What is your walk-in policy?
Walk-in appointments are always welcome!
If you would like to see a therapist without an appointment, patients are encouraged to come to our office during regular business hours:
Monday – Friday 8am – 5pm
Saturday 8am – noon
Sunday Closed
If you or a loved one is having a medical emergency, please call 9-1-1. Patients can contact Vita Fitness & Physical Therapy after the emergency so that we can follow up with your care center for information about our patients.
What should I bring to an appointment?
Patients should make sure to bring the following to their appointment:
- Insurance Card
- Co-Payment
- Doctor referral if needed
- Change of clothes if needed
- Your health questions and concerns
Please arrive 15 minutes before each appointment to allow time for registration.
What insurance is accepted?
We accept most major insurance plans including HMOs and Medicare. We also have special pricing for our patients that do not have insurance. We provide equal access to all patients regardless of insurance status.

How do I access my medical records?
Patients can request their medical records by emailing info@getvitafit.com.
The turnaround time to receive your records is 1-2 weeks.
How can I pay a bill?
Payment for your visit is made during the check-in process before your appointment. We accept cash, check, debit, and several major credit cards.
Patients can also pay their bills by calling or mailing in the payment.
Do I need a doctor to refer me?
Most insurance companies now permit you to get evaluated and treated by a licensed physical therapist without needing a referral or prescription from your doctor.
Check your health insurance plan for more details , or contact us directly if you have any questions.
WHAT TO EXPECT FOR YOUR FIRST APPOINTMENT?
Your first appointment will last approximately one hour and will include a comprehensive evaluation and applicable treatment administered by a licensed physical therapist. Your physical therapist will review your medical history, diagnostic tests and any recent events that may have contributed to your current condition. Your evaluation will include assessment of your current functional deficits, pain level and posture, as well as a thorough evaluation of your flexibility, strength, balance and endurance.
Following your evaluation, your physical therapist will explain your personalized treatment plan and expectations for recovery. Any further visits will be 45minutes.
